Member Enrollment

You’ll need a provider referral to enroll for services with Piedmont Health Partners. Ask your provider to call us at 1-877-906-4478 to refer you to our services.

Piedmont Health Partners can join your care team if you already have other medical care services. We’ll partner with them to provide the services you need.

As a member, you don’t pay for any of the services we provide to you. Our services are an added benefit to your health care insurance.

Accessing Services

After enrollment, you’ll typically have a visit or call once a week for four weeks with one of our health partners. These nurses, social workers, and other professionals work with you and providers in and out of the hospital to help you get care. 

Living in Anderson County makes you eligible for health partner home visits or phone calls. Residents of surrounding counties (including Georgia) can connect with their health partners by phone. After four weeks, your health partner will determine if you need more visits or calls.

Find a Primary Care Provider (PCP)

Find a primary care provider if you don’t already have one. Call the Piedmont Health Partners 24/7 Helpline at 1-877-906-4478.

Essential Primary Care Focus

If you don't have a PCP, your health partner will make a referral. Establishing care with a PCP is important because they provide:

  • Consistent, personalized care and know your health history, preferences, and needs
  • Regular check-ups and routine screenings, immunizations, and lifestyle advice
  • Care coordination with other specialists for your overall health
  • Ongoing care and treatment for chronic conditions like diabetes, hypertension, or asthma
  • Better health outcomes, longer life, and lower health care costs

If you have heart failure, you can work directly with our specialized CHF navigator to manage your condition and coordinate your care.
