What We Do

Piedmont Health Partners is a clinically integrated network of AnMed physicians, providers, and facilities serving Anderson County and the surrounding communities. The network has 650 physicians and providers at AnMed Medical Center, AnMed North Campus, and AnMed Cannon in Pickens County.

We help coordinate and manage patient services from start to finish. Piedmont Health Partners also improves care by assisting doctors in working together, cutting waste, and making patients healthier.

Our priority is encouraging enrolled patients and employees (members) to take an active role in their care. If they don’t already have a primary care provider, we help them partner with one. Whole-Person Care Coordination

Our provider care coordination model emphasizes whole-person care. This patient-centered approach considers the body and mind when developing care plans to provide physical, behavioral, and social support.

Coaching Patients to Engage in Their Health Care

The primary goal of health coaching is for patients and caregivers to become comfortable self-managing their condition(s) and participating in health care decisions.
