Network Patient Referrals

A scheduling coordinator receives and manages all referrals to Piedmont Health Partners. If we need more information about your referral, we’ll contact you to discuss the patient’s needs.

Care Coordination Among Network Providers

You’ll get support navigating patient care between you and primary care providers or specialists. Ask us to set up joint office visits with a clinician at Piedmont Health Partners if you or your patients need extra help:

  • Understanding their health needs
  • Dealing with social factors affecting their health

Provider Communication

You may receive messages from us through electronic medical records or a call to your office to discuss patient concerns. To talk with your patient’s assigned health partner, call Piedmont Health Partner's Helpline at 1-877-906-4478.

Proactive Patient Health Support

Count on us to teach your patients and their caregivers about their conditions, medications, and available resources. Your patients and their caregivers learn how to manage illnesses. We work closely with you on your patient’s progress for continuity of care and better outcomes.
