At AnMed Transitional Care Medicine, our team helps you get the care you need after an inpatient hospital stay.
If you’ve been hospitalized, it’s important for you to continue receiving care to maintain your health. Here’s what you can expect with transitional care:
- If you don’t have a primary care provider (PCP) when you leave the hospital, we refer you to one.
- We discuss your needs for follow-up care with you, based on the reasons you were admitted to the hospital.
- Until you can see your new PCP, our doctors offer transition-of-care appointments so you get tests and treatments when you need them.
If you didn’t stay in the hospital, you can still ask us for care while you’re looking for a new PCP for any reason. We’re here to fill any gaps in care and help you understand the need for routine health visits.
