Get help managing your heart failure symptoms when your doctor refers you to AnMed’s Congestive Heart Failure Clinic.
Your care team at this clinic will include a doctor, nurse practitioners, dietitians and nurses. They will recommend a personalized care plan and teach you how to keep yourself out of the hospital.
Device Management
This shared clinic also contains a Cardiac Device Management Clinic run by Device Management Specialists. You can get monitoring and management of your implantable cardiac device when your doctor refers you to a clinic specialist.
